If you are an adult coming in for the first time, please complete all of the forms in RED and the Black form

If you are a parent completing forms for your child's first visit, please complete all of the forms in RED and WHITE.  If your child is over 12, Please have him/her complete the form in BLACK.

If you need for us to contact anyone on your behalf, your child's behalf, or if you are involved with the court in any manner, please complete the form in GREEN.  

Please bring all designated forms into the office with you for your first visit.  Thank you!
