What to expect

Our intake coordinator will make an appointment with you for the therapist that meets your needs by schedule, specialty, insurance, and/or payment type.  You will have forms to fill out before your first visit.  You are welcome to complete the forms at our office or you may download and print them prior to coming in.  Please go to our forms page to complete the necessary forms prior to your visit.  Co-payments or self pay payments are due at the time of service.  Please provide 24 hours cancellation notice, if you are unable to make it to your appointment.



Directions:  

From I-71 and Fields Ertel, take Mason Montgomery Rd North 2 miles, you will pass Deerfield Town Center and Kroger, Two lights after Kroger turn left onto Cedar Village Dr.  You will pass a cluster of small buildings, then you will pass 2 taller buildings (CMC Office Park), and then you will come to Central Parke Blvd. on your right, turn here.  Please turn at your first left once on Central Parke Blvd., into Cedar Village Office Park, continue left and you will see our building number 7567.  Our office is the office closest to the road and on the right, if you are facing the building.
